Provision of Post Diagnostic Support for Dementia
NHS Greater Glasgow and Clyde wants to commission post-diagnostic support for people newly diagnosed with dementia, delivered by Link Workers on a one-to-one, group, and digital basis. Support should be personalised, flexible, and coordinated to help people and their carers live well with dementia after diagnosis.
