Provision of Hypertension and Lipids Management Services
NHS Norfolk & Waveney wants a provider to supply and manage clinical prescribers for hypertension and lipid management as part of a CVD prevention scheme. Prescribers will work with practices and the ICB to identify and manage high-risk patients, using a personalised care approach including medicines and social prescribing.
